New patients can save time during their first appointment by completing the Patient forms prior to their first visit. Please download/print out the New Patient forms and fill in the requested information. You can email us the completed form or bring the form to your 1st appointment.
Insurance
We accept all PPO Dental plans. Click here to see our In-Network Insurance list.
There are over thousands of dental insurances plans. We do our best to help you understand and work with your insurance policy, find out the allowed maximum, any deductible and eligible services. Dental policies can change every year when contracts renew. We encourage our patients to become familiar with their insurance coverage. Changes in eligibility and benefits could result in unexpected out of pocket cost.
At Dr. Ho’s dental office, we gladly submit your insurance claims for you and will fully attempt to help you receive full insurance benefits. Patients are responsible for payments of the services provided. Please remember that an insurance policy is a contract between you, and your employer and the insurance company, and we have no direct relationship with them.
Financial Obligations
At the time treatment options are presented, our office will do its best to estimate your costs and a payment schedule. We accept cash, personal checks and credit cards. If you qualify, an outside agency can also arrange for an extended payment plan. Contact our office if you need to discuss other payment arrangements.
CareCredit® Dental Financing
CareCredit offers a full range of payment plans so you can find one that works best for you. With the popular No Interest Payment Plans* there are no interest charges if you pay your balance in full within the specified time period. Monthly payments can be as low as 3% of your balance. For procedure fees from $1,000 and up, CareCredit offers 24, 36, or 48 month plan options with low monthly payments available.
